All of our in-network affiliations are listed by state below. When you arrive, remember to bring a valid photo ID and your insurance card. You will be responsible for your copay or deductible at the time of service. For your specific copay, coinsurance, or deductible information, please contact your insurance plan s member services. Due to the variation in plans, our billing staff cannot tell you the exact amount of your out-of-pocket cost. Out-of-pocket cost is dictated solely by your insurance plan.
